    Breast Augmentation FAQ’s

    Your breast augmentation journey starts with the consultation. It’s during this time in which you can discuss how you would like your new breasts to look, the type of implant used, the shape, the procedure itself, risks and potential complications, your medical history and aftercare. We’ll also examine the breasts to get an idea of what we can achieve. 

    Your surgeon will help you determine the best size for you by using a 3D simulation app to help you visualise how the size and shape of the implants may look.

    At 111 Harley St., we also provide you with the opportunity to feel the implants and try the implants on to gage an idea of the weight, feel, and how they will look when wearing clothing.

    A breast augmentation is performed under general anaesthesia and may require an overnight stay in hospital.

    The surgeon will make an incision either below the breast, around the areola or under the armpit.

    Working through the incision, the surgeon will lift the breast tissue and skin to create a pocket which is usually either directly behind the mammary tissue or underneath the chest wall muscle.

    The surgeon places the implants centrally beneath the nipples and closes the incisions with stitches.

    After around six weeks, the results of the breast augmentation surgery will begin to be visible. However, this varies from patient to patient, depending on the speed of healing. Over the next several months, the implants will begin to soften and settle into their final position.

    If you’ve opted for under the muscle placement, the breasts may appear slightly higher due to the tension on the pectoralis muscle. The implants will ‘drop and fluff’ after around six weeks as the muscle starts to relax, and the implants shift from the upper portion of the breast to the lower pole.

    Enhancing the appearance of your breasts doesn’t always require a breast augmentation. Depending on your aesthetic concerns, you may require a combination approach to breast enhancement or a single procedure. It’s important to remember that there is not a ‘one-size-fits-all’ approach to breast enhancement, and each pair of breasts is unique and come in various shapes and sizes. Many factors may influence your decision, including, scarring options, implant types and side effects, and overall look and feel.

    If you don’t like the idea of breast implants, fat injections are suitable for small breasts. A breast fat transfer can increase the size of the breasts by 1-2 cup sizes and uses the patient’s fat. However, the operation does come with its risks and side effects such as lumpiness, mammogram implications, and fat reabsorption.

    Recovery varies from patient to patient, but we encourage patients to be as healthy as possible before surgery to accelerate the speed of healing.

    You may wish to consider taking a week or so off work and arrange help with driving, childcare etc. The general recovery time is six-eight weeks.

    The duration of the procedure varies on the extent of the surgery itself. If you require a breast lift in addition to breast implants, this will take longer. Typically, it takes around 1 ½ hours to 3 hours perform the procedure.

    Side effects and risks include (but are not limited to):

    • Loss of sensation on the nipple and breast
    • Puckering of the breast (ripples)
    • Asymmetry
    • Creasing or rotating of the implant
    • Extrusion (implant becomes visible through skin)
    • Bruising
    • Infection
    • Redness
    • Swelling
    • Scarring
    • Inflammation
    • Blood clot and bleeding
    • Soreness and pain
    • Necrosis
    • Palpability (feeling implant through skin)
    • Stitching concerns
    • Wound separation
    • Implant rupture, deflation and leakage
    • Capsular contracture (causing a misshapen appearance)
    • Bottoming out (implants drop below natural breast crease)
    • Double bubble effect (implant sits above natural breast tissue)

    If you experience any complications following a previous breast enlargement surgery, or depending on the severity of your concern, you may require a replacement or revision surgery.

    No matter how skilled the surgeon, there will always be some scarring left behind when performing a breast enlargement procedure. You can discuss the incision placement with your surgeon before proceeding. If the incision is placed under the armpit or around the areola, scarring will be less visible. In any case, scarring should fade significantly over two years. At 111 Harley St., we’re one of the few UK clinics to offer the transaxillary incision (through the armpit).

    If scarring is of great concern with you, we recommend iPixel scar revision technology or Endermologie as a treatment plan as part of recovery. Scar revision is also a surgical option.

    Surgical Techniques

    One favoured Breast Augmentation method involves using the Keller Funnel. The Keller Funnel has some resemblance to an icing piping bag and means the implant can be placed within an incision half it’s size. Another benefit of using a Keller Funnel is that it means the implant remains sterile, intact and ready for seamless placement – however, the biggest advantage of this method is that the incision required is very small.     Other Incision Types

    UNDER THE BREAST

    The surgeon makes an incision under each breast, sometimes referred to as an under the breast implant, where the implant itself is inserted where the breast meets the chest. This incision type works well for those who have large breasts, as the scar can be concealed in the crease.

    AROUND THE AREOLA

    Making an incision around the areola is a virtually a scar-free option. The incision point is barely visible due to the colour and texture of the areola. It’s preferable for those who need a lift.

    UNDER THE ARMPIT

    The surgeon makes the incisions under the armpit. This type of under the armpit breast implants means any residual scarring is not visible when the arms are elevated.

    Implant Shape

    The implants we endorse at 111 Harley St. are Motiva. Motiva is a brand committed to helping women feel their best through their range of ergonomic implants that mimmic the feel of real breast tissue. The Motiva Ergonomix implant is a hybrid implant, and can come in all different shapes and sizes.

    ROUND

    • Rotate freely, so will not change shape if the implant turns.
    • Rounder appearance.
    • Can help achieve more fullness in the upper pole of the breast.
    • May look more artificial.

    TEARDROP

    • More natural appearance of the breast shape.
    • Might give preferable results for flatter chested women.
    • Slight increased risk of rotation of the implant.

    Implant Type

    Your surgeon will discuss the best implant type for your needs. Dr Yannis Alexandrides favours the use of Motiva implants and doesn’t use saline implants in our practice. They can look like a teardrop-shaped implant when standing but look rounder when lying down. They provide very natural-looking results and are made from ProgressiveGel Ultima Silicone Gel and can offer many benefits that traditional implants cannot.

    SILICONE

    PROS

    • More natural look and feel.
    • Less likely to rupture.
    • Less rippling and wrinkling.

    CONS

    • Though rupture of silicone implants is unlikely, if it does occur, it can often go unnoticed and cause pain in your breast or change the shape of them.
    • More of a long-term risk.

    MOTIVA

    PROS

    • Can narrow cleavage or give a cleavage boost.
    • Are suitable for breast reduction (with implants) when the patient desires lifted-looking breasts that are not deflated.
    • Preferable for those that want natural-looking breasts.
    • Suitable for smaller chested women.

    CONS

    • Some patients don’t like the idea of the microchip technology in the implant, but you do have the choice to opt out. It can be useful for collecting medical data or if you require a re-operation.

    Other Breast Augmentation Options

    Fat Transfer To The Breast

    A fat transfer extracts fat through liposuction. The surgeon will purify the fat and re-inject it back into the breasts. Since the fat is a natural substance taken from the patient’s body, it’s a more natural approach to breast augmentation that carries fewer risks.

    Fat transfer to the breasts is generally suitable for women who have excess fat (that may be residing in undesirable areas) and are looking to increase their breast size by 1-2 cup sizes. Fat can be removed from areas like the thighs, flanks, bra rolls, hips or any other areas. It’s an ideal option for those looking to take advantage of the body sculpting benefits that this operation offers, by distributing fat in a more aesthetically-pleasing way.
